I have accepted the Lord Jesus Christ as my personal Savior and believe that I am born again.

I desire to be obedient to the command of the Lord to be baptized in water in the name of the father, the son, and the holy spirit (Matt. 28:19).

I understand that baptism by immersion represents burial and resurrection with our Lord Jesus Christ (Romans 6:4).

I understand that this baptism is not the moment of my salvation, nor equivalent to joining this church, but rather a public declaration of my decision to follow Christ (1 Peter 2:21).
